No Pain.

Pain management is a challenging and complicated subject, whether you’re getting back on the field after a sports injury, rehabbing after a workplace accident, or dealing with chronic issues like arthritis or multiple sclerosis. No method of pain management is efficient for everyone, and even the tried-and-true approaches aren’t without their problems.

Over-the-counter pain relievers like NSAIDs can be efficient in the short term but carry significant health risks in the long term. Ultra-distance athletes, for instance, are advised against using NSAIDs during long training sessions to avoid renal failure, and anyone who uses them frequently for long periods of time is at increased risk of heart attack and stroke.  Opioids do a great job of relieving pain—and are also the source of an addiction epidemic that kills tens of thousands of people in the U.S. every year.  A way to treat pain without serious side effects and without the risk of addiction could be a godsend for athletes and, frankly, anyone who ever hurts.

Enter cannabidiol, more commonly known as CBD. It’s a (non-intoxicating) derivative of the cannabis plant that works with the endocannabinoid system,  which helps to regulate physical processes  ranging from pain perception to bone density to hunger response. Some athletes report cutting back or even cutting out the use of NSAIDs without serious side effects after trying CBD . And while the effects of CBD might not be as definitive as opioids at altering intense, acute pain, it could be efficient as part of a regimen with other meds to manage long-term pain.

The Science Behind CBD

CBD may activate pathways that reduce pain and inflammation by working on receptors in your brain that affect the central and peripheral nervous system. It may stimulate receptors that control numerous aspects of your brain’s pain control systems as well as receptors in the body’s soft tissues that manage pain and inflammatory processes. It might also slow the reuptake of adenosine, a neuromodulator that influences, among other physiological functions, inflammation. More adenosine in the nervous system means less inflammation and can even help achy athletes get a better night’s sleep.

A 2008 review of research studies evaluated the efficiency of CBD against chronic pain. The suggested conclusion was that CBD shows great promise as a potential future treatment for pain as well as favorability for insomnia related to chronic pain based on the results of multiple clinical trials. Back in the lab, a 2016 study tested the effect of CBD gel on rats with arthritis. When the gel was rubbed on the rats’ tiny little arthritic joints, researchers saw a decrease in behavior indicating inflammation and pain. A 2018 report in the journal Pediatric Dermatology even noted three children with epidermolysis bullosa—a rare disorder that causes fragile, blistering skin—who saw faster wound healing and pain reduction after topical application of CBD. During the study, one individual stopped their opioid pain medication entirely. In any case, these results are far from conclusive—more research needs to be done.

While CBD has shown potential to reduce pain and inflammation in research settings, it isn’t a miracle cure—sometimes, pain is your body’s way of telling you something is terribly wrong. Use common sense, listen to your body, and if you suspect you have a significant injury, take it to your doctor.
